Dear Parents, 

For the upcoming school year, all student medical information will be in an online database. There are many benefits to this database including less paperwork for parents to complete each year and having more accurate information that is accessible to parents and the appropriate employees at our school. The management system we use is called Magnus Health SMR (Student Medical Record). As this is a web-based system, you will have continuous access to your child’s health record as well as the ability to make updates when needed. You will also have the option to access the account after your child graduates.

You now have access to your Magnus Health SMR account through the Veracross Family Portal on the GCDS website. You will access the account via the button titled “Magnus Health.” We ask that you enter the health information required within your Magnus Health SMR account for each child attending GCDS no later than August 1st.

***Physical exams are now required every year for ALL students.  Students who participate in sports must have a current physical exam on file before the start of pre-season (Upper School) and before the start of the fall season (Middle School).  Please upload your child’s physical exam into their health portal, do not send hard copies of their physical to the school.***

Timely submission of health information and other forms is vital so that preparations can be made prior to the first day of school. You will receive weekly reminders as long as there are outstanding items in your account.
